It notes the advent of anatomical radical prostatectomy, the introduction of prostate-specific antigen (PSA) and its consequences. It notes on the adverse side the production of a wholly iatrogenic stage of the disease; non-metastatic, hormone-resistant prostate cancer; the overdiagnosis and overtreatment of patients, with the marked reduction in postmortem incidentally found prostate cancer as part of this phenomenon. By use of case studies it demonstrates that the course of prostate cancer is frequently more prolonged than usually acknowledged and significant progress in managing the disease has been made. The academic and clinical community researching and managing the disease is still confronted with an increase in incidence of prostate cancer in the United States, for example, from 101 000 cases in 1980 to an estimated 238 590 cases in 2013, due mostly to PSA testing, the actual numbers of deaths ascribed to prostate cancer in 1980 was 31 000, whereas the estimated number of deaths in 2013 is 29 720, suggesting that most of the men at risk of dying from the disease, 20 years into the PSA era, are either not receiving timely, curative therapy, or the current approach to management of the disease is not being effective in those who most need it." @default.
