• W1565161941 abstract "OBJECTIVE: We evaluated the relationship between iron and cognition in several deep gray matter (DGM) structures while accounting for the influence of volume loss in multiple sclerosis (MS). BACKGROUND: DGM atrophy, a sign of neurodegeneration, and cognitive dysfunction are correlated in MS. However, the cause of neurodegeneration in MS is poorly understood. Because iron accumulation in the DGM is increased in MS compared to age- and sex-matched healthy controls, it may contribute to disease progression. DESIGN/METHODS: Eighty-five MS patients underwent magnetic resonance imaging (MRI) and neuropsychological examination. We used susceptibility-weighted imaging (SWI) filtered phase to analyze the mean phase of low phase voxels (MP-LPV), indicative of abnormal iron accumulation. RESULTS: Correlations between MP-LPV and cognition in MS were found in the caudate nucleus (R=0.240, p=0.030 Symbol Digits Modalities Test (SDMT); R=0.232, p=0.036 Paced Auditory Serial Addition Test 3s (PASAT3)), putamen (R=0.368, p=0.001 SDMT; R=0.252, p=0.022 Delis-Kaplan Executive Function System Sorting Test-Correct Sorts (DKEFS-CS); R=0.238, p=0.031 Brief Visuospatial Memory Test-Revised, Total Learning (BVMTR-TL)), globus pallidus (R=0.235, p=0.033 BVMTR-TL), and pulvinar nucleus of thalamus (R=0.244, p=0.027 SDMT; R=0.255, p=0.021 DKEFS-CS; R=0.251, p=0.023 BVMTR-TL). However, correlations between structure volume and cognition were more robust than correlations between MP-LPV and cognition, and the introduction of structure volume into hierarchical regression analyses after iron metrics significantly improved most models. Furthermore, MP-LPV did not account for significant variance after structure volume when predicting cognitive performance. CONCLUSIONS: These findings suggest that iron accumulation plays a significant, if minor, role in MS cognitive decline. This use of SWI technology strengthens previous findings of cognitive relationships with iron accumulation using T2 hypointensities, R2*, and magnetic field correlation.
