This chapter discusses febrile non-haemolytic transfusion reactions (FNHTRs) and both mild and severe forms of allergic reactions. It summarizes the current understanding of the mechanisms causing FNHTRs with red cells and platelets. As the pathogenesis of FNHTRs is different for red cells and platelet transfusions, the strategy for their prevention depends on the blood component being transfused. Premedication of the patient with an antipyretic drug, paracetamol in the UK and acetaminophen in North America, has become standard practice to prevent FNHTRs. The management strategy for FNHTRs requires clinical judgement and must balance the benefits and risks of their investigation and treatment. The management strategy for FNHTRs associated with red cell transfusions should include an approach to rule out an acute haemolytic transfusion reaction." @default.
