- W1567152185 abstract "Despite recent progress in our knowledge of the mechanisms and management of congestive heart failure (CHF), mortality remains high in patients with this condition. ACE inhibitors have substantially reduced mortality in symptomatic CHF patients (1–3). However, the benefit provided by this class of agents reaches its peak within a few months after initiation of therapy [1]. More recently, beta-adrenergic blockade has been shown to improve survival when added to ACE-inhibitor therapy in patients with CHF [4–13]. Nevertheless, mortality from progressive heart failure remains high despite the use of such agents. Thus it appears that attenuation of neurohumoral pathways may not be sufficient to allow complete control of the remodeling process in these patients. This chapter briefly reviews compensatory neurohumoral mechanisms as well as mechanisms potentially related to inflammation, apoptosis and oxidative stress in chronic congestive heart failure.
