- W1567201405 abstract "Objective: To compare the performance characteristics of the WHO Ten question screen in HIV+ vs. HIV- children in Botswana.Background: The World Health Organization “Ten Question Screen,” a short questionnaire given to parents, is the most commonly used screening tool for the detection of neurologic and developmental disorders in low-resource settings. It has been shown to have good sensitivity and adequate specificity in the general population of children in Sub-Saharan Africa. However its performance characteristics in children with HIV infection, who may have a different distribution of cognitive and motor impairments than the general population, is unknown.Methods: We prospectively enrolled 137 children between the ages of 2-16 (22 with HIV infection and 115 children without HIV) from Princess Marina Hospital in Botswana to determine the performance characteristics of the Ten Question Screen in detecting motor impairment, cognitive impairment, hearing impairment, visual impairment, and seizures.Results: In HIV uninfected children, the Ten Question Screen had excellent sensitivity and specificity for motor impairment (Sensitivity 89[percnt], Specificity 99[percnt],) cognitive impairment (Sensitivity 91[percnt], specificity 99[percnt]), visual impairment (85[percnt], 93[percnt]) and seizures (87[percnt], 92[percnt]). Sensitivity for hearing impairment (33[percnt], 93[percnt]) was poor. Performance characteristics were similar in HIV infected children except for detection of cognitive impairment, in which sensitivity was poor (Sensitivity 50[percnt], Specificity 90[percnt]).Conclusions: The Ten Question Screen has good performance characteristics for the detection of motor and cognitive impairment in a general population of children in Botswana. A two-stage process with confirmatory testing is necessary for the detection of hearing impairment. Formal cognitive testing is necessary for the detection of cognitive impairment in children with HIV due to a relatively high prevalence of mild cognitive impairment in this population that goes undetected by the Ten Question Screen.
