- W1567358281 abstract "Publisher Summary This chapter presents the guidelines for optimizing symptom control in patients with amyotrophic lateral sclerosis (ALS). Dysphagia is a common problem in ALS. Because eating and drinking is not only a basic physiological function but also has a great social impact, dysphagia is a very distressing symptom for most patients. It is very important to reassure the patients that whenever they decide to stop non-invasive ventilation (NIV), all necessary care and appropriate medication will be available to ensure a peaceful death. Dyspnea is most distressing symptom in ALS. In patients not receiving mechanical ventilation, the feeling of shortness of breath can be reduced by the administration of morphine. Titration of the morphine dose against the clinical effect will almost never lead to a life threatening respiratory depression. Recent study indicated that spirituality or religiousness may affect the use of percutaneous endoscopic gastrostomy (PEG) and NIV in ALS and may be a source of comfort to the patients. Cases of patients whose spiritual practice greatly enhanced their ability to cope with ALS have been reported. A simple structured interview to assess the patient's spiritual needs has been recently developed." @default.
