- W1569429535 abstract "To the Editor: Ahmed and colleagues deserve applause for describing and assessing the merits of their new program in geriatric education at the University of Texas Medical School at Houston.1 We did note, however, that the curriculum offered little or no experience in the care of older adults in the intensive care unit (ICU). With the number of older individuals treated in such settings having almost doubled over the past 10 years,2 might this not be an appropriate setting as well for an educational experience? Older adults (≥65) account for more then 50% of ICU admissions and for almost 60% of all ICU days,3 so internists and the full range of subspecialists, such as cardiologists and pulmonologists, who provide large amounts of care in that setting should be knowledgeable about geriatric medicine. Should not such specialists have a full appreciation of the physiology of aging and how to take into consideration an individual's functional capacity when an older adult is in the ICU and during the time before the acute illness? Only then can an improvement or lack of improvement be adequately assessed. In addition, many symptoms and signs, for example, abnormal mental status and limited functional capacity, mimic those observed in large numbers of older adults not in that location. It is likely that knowledge about geriatric medicine would also be of assistance when talking to the families of critically ill older adults who appeared not to be recovering from serious acute illnesses. As noted in a front page article in the Wall Street Journal, marketers are learning how to market to an ever-enlarging population of “aging boomers.”4 Should not all physicians, including intensivists, also be knowledgeable about geriatric medicine?
