Unilateral renal arterial disease, though less commonly associated with hypertension than is unilateral pyelonephritis, has been associated with a much higher rate of cured hypertension following nephrectomy. The general opinion, as reflected by Thompson and Smithwick,2 is that renal arterial lesions represent the closest clinical counterpart to the experimental Goldblatt kidney.Characteristically, patients with renal arterial disease have an abrupt onset of hypertension.
