- W1570359634 abstract "Thank you for the opportunity of replying to the commentary from Siffledeen et al.,1 who have recognised the importance of our work in defining mortality to be an area of on-going concern, not only in Crohn’s disease2 but also in ulcerative colitis.3 In Crohn’s disease, we show increased age-related mortality, not just in the ‘elderly’ but also in patients aged 50–64 years. This is new information with important implications for patients and society. We acknowledge the well-recognised limitations of database research, in particular, the difficulties in evaluating factors influencing clinical decision-making in a retrospective analysis. Our findings come from a long established data collection system that has been validated for many diseases and is professionally managed and subject to audit and reconciliation. The ISD datamart ACaDMe (Acute Cancer Deaths and Mental Health) encompasses the linked in-patient and daycase (SMR01), mental health (SMR04), cancer registration (SMR06) and death (NRS) records from 1981 to present day. It holds records of all NHS hospital admissions for the entire population of Scotland (∼5.2 million), with linked individual patient hospital admission data including diagnosis, comorbidity, procedures, deaths, hospital, health region and social deprivation. We strongly endorse Siffledeen and colleagues’ comment regarding the importance of these data as a comparator for the outcome of patients treated in the era of biologicals. Indeed, the greatest value of our study may be the provision of a benchmark for evaluation of mortality in Crohn’s disease since their introduction in Scotland.
