- W1572198977 abstract "OBJECTIVE: To describe seizures characteristics, antiepileptic drugs (AED) used and prognostic considerations in patients with cancer and seizures BACKGROUND: Patients with cancer develop seizures with a frequency of 4-13%, scant information regarding seizures in patients with cancer is found DESIGN/METHODS: From 2010 to 2012 all patients with cancer and seizures, at a referral Cancer institution, were included in the study. Age, gender, seizure type, seizure etiology, AED employed, efficacy of AED and prognostic considerations were analyzed RESULTS: The present series describes 210 patients with seizures and cancer, 71% had seizures after the diagnosis of cancer, in 10% seizures led to the diagnosis of cancer. Of all the patients, 81% (n 172) had seizures associated to cancer or its treatment (SAC). Seizure type in patients with SAC included partial onset 56%, generalized 31%, mixed and others 13%. Etiologies of SAC were: Brain metastases 48%, primary brain tumor 11%, stroke 10%, cardiorespiratory conditions 6% and infectious condition 6%; only 2 patients had seizures due to chemotherapy. The most common single AED used was phenytoin (PHT) 33% followed by oxcarbazepin (OXC) 16%, valproic acid (VPA) 11%; combination therapy was used in 17%. Efficacy of AED (less than 1 seizure per year) was 100% for lamotrigine (LMT), 57% for OXC and carbamazepine (CBZ), 47% for VPA, 46% for PHT, 33% levetiracetam. Adverse effects grades III-IV were seen in 18 patients, 8 due to PHT. Among patients with CAS, status epilepticus was seen in 24 (14%) patients and a single seizure in 52 (30%). Mortality among the whole series was 27% (n 47), highest for those who had SE (60%) CONCLUSIONS: Seizures in patients with cancer is a frequent comorbid condition with a high mortality rate, further studies are required to study the efficacy and security of AED and other forms of treatment.
