- W1573319372 abstract "To evaluate the correlation between sonohysterographic findings and histology, considered as standard reference, for the diagnosis of endometrial polyps. We retrospectively reviewed records of 254 patients, suspected to present endometrial polyps at transvaginal ultrasounds and subsequently submitted to saline infusion sonohysterography (SIS), from January 1st 2009 to December 31st 2011. Histopathological samples were obtained by office endometrial biopsy using Pipelle sampler after SIS or during operative hysteroscopy. Sensitivity, specificity, positive and negative predictive values (PPV, NPV) were calculated to assess the accuracy of SIS compared to histology. Median age was 44 years. 37 patients were postmenopausal (15%). Out of 254 SIS reviewed, 144 recorded a diagnosis of endometrial polyp (56.7%) of which, 52 had a diameter ⩽ 1 cm (36.1%), 77 had a diameter between 1 and 2 cm (53.4%) and 15 had a diameter ≥ 2 cm (10.4%). 44 patients showed more than one polyp (30.5%). 9 patients were excluded because of the lack of histological sample (8.1%). Analysis of the samples showed: 90 proliferative endometrium or atrophy, depending on patient's age (35.4%), 139 polyps (54.7%), 5 simple hyperplasia (2%), 2 complex hyperplasia (0.8%), 6 simple hyperplasia and concomitant endometrial polyp (2.4%) and 3 complex hyperplasia and concomitant endometrial polyp (1.2%). In 4 patients (2.8%) operative hysteroscopy (performed 6 months after sonohysterography) showed a regular uterine cavity and concomitant endometrial biopsy was negative for polyps. SIS showed a sensitivity of 96%, specificity of 96% and PPV of 97% and NPV 94% for polypoid lesions. SIS shows an higher accuracy in diagnosis of endometrial polyps compared to transvaginal ultrasounds; the high PPV allows to select patients who should undergo operative hysteroscopy, while high NPV allows to avoid invasive diagnostic methods." @default.
