- W1574137131 abstract "Ultrasound (US) and fetal magnetic resonance imaging (MRI) have important complementary roles to play in the diagnosis of prenatal structural brain abnormalities. While US has the benefit of real time imaging and transvaginal, transfontanellar neurosonography improves visualisation of the neural axis, MRI has superior sensitivity for cortical abnormalities and resolution of structures in the posterior fossa. Here we describe a case of evolving unilateral cerebellar hypoplasia diagnosed in the midtrimester subsequently found at postnatal MRI to be due to venous infarction. A 34 year old G2P1 woman had an uneventful early pregnancy and first trimester combined screening. At the time of her midtrimester morphology scan, she was noted to have mild unilateral cerebellar hypoplasia. The vermis was of normal size and morphology and the remainder of fetal anatomy appeared normal. Normal karyotype was confirmed with amniocentesis. An MRI performed at 24 weeks confirmed unilateral cerebellar hypoplasia with no associated pontine or vermian abnormality. The supratentorial structures appeared normal, and there was no evidence of haemorrhage. The patient was counselled about the significant risk for developmental or speech delay, autistic features, ataxia, hypotonia, and ocular signs, but elected to continue the pregnancy. The cerebellar hypoplasia evolved during the course of the pregnancy, with marked asymmetry seen on US at 30 weeks. There was no other abnormality seen, and there was no secondary ventriculomegaly. Post natal neurological examination was normal. Post natal MRI was performed at 2 days of age showed marked left cerebellar hypoplasia with an associated venous sinus thrombosis within the adjacent left transverse and sigmoid sinus, suggesting to the aetiology was a left cerebellar venous infarct. The remaining intracranial structures appeared normal. This case demonstrates the complementary roles of US and MRI for complex neurological developmental abnormalities." @default.
