- W1575083430 abstract "While in the USA more than 70% of radical prostatectomies are performed with robotic assistance (RALP), in Europe laparoscopy (LRP) covers up to 25% of the procedures and is a well-established procedure for the treatment of localized prostatic cancer. There are two main approaches: transperitoneal and extraperitoneal. Although the former is mainly adopted for RALP, the latter is preferred for pure laparoscopy. Both approaches can be performed in two different fashions: antegrade (descending; most common) and retrograde, which essentially retraces the steps of radical retropubic prostatectomy (RRP). Several randomized controlled trials, observational series, and reviews have compared LRP to RRP or RALP. Increased operative times and a longer learning curve are the criticisms against LRP, even if the extraperitoneal approach has shown improved performance. LRP has demonstrated a significantly lower blood loss and transfusion rate and shorter hospital stay. The first long-term oncologic results are already available and show comparable positive surgical margins, biochemical failure rate, and clinical progression-free outcome at 10 years. Few reports compare functional results: referral centers have shown overlapping continence and erectile function rates, although a there seems to be a significant trend in favor of RALP. Even if RALP has solved most of the ergonomic deficiencies of LRP, technologic advances of videoendoscopy (i.e. high definition, three-dimensional) and new robot devices and platforms, which allow the surgeon to sit over the patient's head, allow excellent results to be achieved with some cost savings of costs, although there are still comparatively high." @default.
