Cardiac arrhythmias continue to present a significant medical problem. One type of arrhythmia, namely ventricular fibrillation, is the biggest single cause of death in many developed countries. The electrophysiology of normal cardiac tissue, and how its disturbances result in arrhythmias, is discussed, as are the molecular targets for current and potentially new antiarrhythmic drugs. The search for new drugs is important since, despite the marked advances that have been made in treating arrhythmias with implanted electrical devices, they are not universally applicable to all arrhythmias. Many current and potential future antiarrhythmic drugs act primarily by virtue of blocking cardiac ion channels with different classes of drugs acting on sodium, potassium, or calcium channels. They specifically target a particular type of channel or less specifically act simultaneously on different types of ion channels. However, over-expression or inappropriate expression of such actions produces arrhythmias to impose inherent limitations to their usefulness. Attempts to block the mechanisms responsible for the genesis of arrhythmias have not been very successful, although new potential targets involving such mechanisms are constantly being discovered.
