Sacral functions is used as a cover term, “pelvic floor dysfunction” for disorders of striated muscle. Neurophysiological tests have been proposed to strengthen and refine a clinical diagnosis of neuromuscular involvement in patients with sacral dysfunction. This chapter discusses some tests in selected patient groups (or clinical settings) in which uro-neurophysiological tests (UTs) are of practical relevance to the clinician. Apart from being performed on a highly personal part of the body, there are anatomical, (patho) physiological and technical specificities to be considered. The pros and cons are mentioned. In addition to factors influencing EMG parameters of any muscle, external anal sphincter (EAS) reference values could possibly be influenced by chronic constipation, and vaginal deliveries. The characteristic patterns of motor unit potential (MUP) and interference pattern (IP) parameter changes are expected in different groups of patients, according to the underlying pathological processes. A recent WHO consensus statement has recommended that UTs be performed within or in close collaboration with clinical neurophysiological laboratories. Review on UT application in patients with incontinence has stated that they may be considered in patients with suspected or known peripheral lesions. Concentric needle CNEMG has been reported to be of diagnostic significance to women with urinary retention and obstructed voiding. Intraoperative sacral neurophysiology might prove helpful in surgical procedures involving the sacral neuromuscular system itself or closely related (neighboring) structures—particularly traumatic and other lesions involving the spinal canal at the thoracolumbar junction." @default.
