- W1590944181 abstract "This chapter discusses the role of functional magnetic resonance imaging (fMRI) in identifying the eloquent cortical areas. The most commonly used contrast mechanism employed in fMRI is blood oxygen-level dependent (BOLD) contrast. BOLD contrast reflects a complex interaction between blood flow, blood volume, and hemoglobin oxygenation. BOLD-fMRI benefits greatly from high-speed imaging, both for the acquisition of temporal information, and for the reduction in the effects of physiological noise and head movement. fMRI has the advantages that it can produce functional and anatomic maps concurrently and preoperatively, and is equally sensitive to superficial and deep regions. It is noninvasive, cost-effective, and easy to employ even using the routine clinical scanners. Cortical reorganization and participation of ectopic neuronal tissue in physiologic cerebral functions are of clinical importance in patients who are considered for epilepsy surgery with resection of cortical dysplasia. In addition to localizing the physiological activation, fMRI has the potential to identify epileptogenicity of dysplastic cortex by imaging ictal or interictal events." @default.
