She had been diag-nosed with rapidly progressive glomeru-lonephritis in 1966 leading to completerenal failure in 1973. Treatment includedmultiple failed attempts at transplanta-tion and long-term hemodialysis. Shewas diagnosed with beta-2 microglobulinamyloidosis in 1986. She had a para-thyroidectomy for secondary hyper-parathyroidism, a history of renal osteo-dystrophy, multiple surgical releases andtenosynovectomies for bilateral carpaltunnel syndrome and trigger finger, andpolyarthritis. Radiographs demonstrateda large lytic defect in the right femoralneck. A smaller lytic defect was noted inthe left femoral head. As prophylaxis, thepatient underwent internal fixation of theright hip with a dynamic hip screw(DHS). Curetted bone taken from thelesion at the time of surgery stained posi-tive with Congo red and exhibited thecharacteristic dichroism of amyloid. Im-munoperoxidase stains for beta-2 mi-croglobulin were also positive.Six months postoperatively, hip dis-comfort persisted and the patientremained unable to fully weight-bear.Despite a well-fixed pin and plate, thelarge lytic defect in the right femoralneck was unresolved, and no signs ofhealing were evident on radiography. Inaddition, she exhibited diffuse myofascialpain and muscle spasms. This pain wasdistinct from her bony pain and was es-pecially prevalent in her pelvic girdle.Physiotherapy as well as narcotics, mex-iletine, benzodiazepines and systemic xy-locaine were tried with some success. Se-lective serotonin reuptake inhibitors(SSRIs) and antiepileptics were used for apainful femoral neuropathy.In June 2002, the patient developedsevere left hip pain, and radiographs re-vealed an incomplete fracture throughthe previously identified lytic defect.Open reduction and internal fixationwith a DHS was performed. In April2003, the patient experienced a furtherincrease of her longstanding right hippain. Radiographically, marked angula-tion at the inferior aspect of the prosthe-sis was seen and was attributed to frac-ture of the proximal femoral shaft. TheDHS was removed, and revision with alocked femoral antegrade intramedullarynail was undertaken. Biopsy showed noevidence of amyloid at this fracture site.Investigation of severe right thigh painpostoperatively revealed an intramuscularamyloid deposit, palpable on physical ex-amination. Radiographs in January 2004showed evidence of healing around theright femoral shaft fracture. Fracture linesat the left hip did not show signs of heal-ing, but at the right hip, the fracture linesshowed early callous.
