- W15975790 abstract "Vancomycin was used to treat serious Gram positive infections for thirty years before significant resistance to its activity developed. In 1986 vancomycin-resistance was described in two clinical isolates ofEnterococcus faeciumfrom France (1). Fourteen years later, vancomycin-resistance in enterococci is a worldwide problem; vancomycin-resistant enterococci (VRE) have been isolated in 18 countries on six continents. VRE are a particularly serious problem in the United States. In 1998, over 20% of enterococcal isolates reported from the Center’s for Disease Control and Prevention (CDC’s) National Nosocomial Infection Surveillance (NNIS) System hospitals were resistant to vancomycin, an increase of more than 50% compared to the period 1993–97 (2). The steady rise in the number of enterococci resistant to vancomycin shows no sign of slowing, despite the institution in many US hospitals of 1995 federal guidelines for decreasing the spread of VRE (3). This trend is worrisome for several reasons. First, the majority of vancomycin-resistant enterococcal isolates are concomitantly resistant to moderate-or high-levels of penicillins and to high-levels of aminoglycosides, often relegating treatment to unproven combinations of antibiotics or to investigational compounds (4). Even Synercid®(quinupristin/ dalfopristin), an agent recently approved in the US for the treatment of vancomycinresistantE. faeciuminfections, has a reported efficacy of only 52.3% (5).
