- W1599311036 abstract "526 Objectives We compared diagnostic performances for differentiating recurrent glioma from post-treatment change of PET/CT scans using F-18 FDG (FDG), F-18 FLT (FLT), and F-18 FET (FET). Methods A total of 22 consecutive patients with suspicious recurrent glioma underwent FDG, FLT, and FET brain PET/CT scans. Maximum SUV of tumor and contralateral normal brain region were measured respectively, and their ratio was denoted as SUV ratio. Tumor recurrence was verified by histologic confirmation or clinical follow up. SUV ratios of each PET/CT dataset were compared between patients with confirmed recurrent tumor (recurrence group) and post-treatment change (non-recurrence group). ROC curve analyses for recurrence were performed by using SUV ratios of each PET tracer. Results Fifteen of twenty-two patients (68%) were classified as a recurrence group. SUV ratios of FLT had the tendency to be higher in recurrence group compared to non-recurrence group (median, 2.8; range, 1.0-8.2 vs median, 2.1; range, 1.1-2.7, p = 0.078). However, there were no significant differences in SUV ratios of FET (p = 0.27) or FDG (p = 0.65) between two groups. ROC curve analyses revealed that the AUC, optimal criteria, sensitivity, specificity, and accuracy were 0.738, >2.1, 80%, 57%, and 73% for SUV ratio of FLT, 0.648, >1.6, 73%, 71%, and 73% for FET, and 0.569, >0.6, 73%, 57%, and 68% for FDG, respectively. Based on Multi-ROC curve analyses, the combined criteria-SUV ratio of FLT >2.1 or SUV ratio of FET >1.6-showed a higher accuracy (82%) for identifying the recurrence with the sensitivity and specificity of 88% and 67%, respectively. Conclusions FLT PET is superior to FDG or FET PET in differentiating the recurrent brain tumors from post-treatment change. Combination of FLT and FET PET might improve the diagnostic performance." @default.
