- W1600938046 abstract "OBJECTIVE: To estimate the prevalence of social phobia (SP) in early Parkinson9s disease (PD), as well as to analyze the relationship between the severity of SP and PD symptoms in PD patients. BACKGROUND: Many psychiatric symptoms such as generalized anxiety disorder, panic disorder and social phobia are present in PD. SP is more common among PD patients than in the general population. SP may not only be explained by psychosocial mechanisms of PD but by a neurobiological mechanism underlying PD. SP may be due to decreased dopaminergic function in the striatum which in turn may be implicated in the pathogenesis of SP in PD.DESIGN: The study consisted of 41 subjects diagnosed with PD for under 5 years. Patients with impaired cognitive abilities, advanced disease and motor on/off fluctuations were excluded as anxiety may be a manifestation of off-phenomena. UPDRS and LSAS scales were used to evaluate the degree of PD and SP symptomatology, respectively. They were compared statistically by Pearson correlation coefficients followed by the non-parametric Spearman’s Rho as many of our samples were small. The ratings were also analyzed in relation to UPDRS subdivisions I, II, and III. A sub-analysis was also done for: patients on medications that can treat SP symptoms, treated vs. untreated PD patients, depressed vs. non-depressed PD patients.RESULTS: There was not a significant correlation between UPDRS-III and LSAS. Between LSAS and UPDRS-I (evaluates non-anxiety psychiatric symptoms in PD), the correlation was significant and even higher in the untreated PD subgroup suggesting dopamine replacement may be needed to treat SP symptoms in PD.CONCLUSIONS: The high correlation between LSAS in the untreated PD subgroup suggest the severity of SP symptoms in PD may be associated with brain regions linked to mood and memory, rather than an individual reaction to the motor symptoms of the disease. These results also suggest SP symptoms might respond to PD dopaminergic replacement drugs instead of the conventional SP treatment. Disclosure: Dr. Saldivar has nothing to disclose.
