Objetivos: Describir el comportamiento del empleo de transfusiones sanguineas y de hemorderivados y las complicaciones asociadas en el Instituto Nacional de Nefrologia de enero de 2005 a enero de 2010. Material y Metodo: Se realizo un estudio observacional, descriptivo, de cohorte retrospectiva. El universo de estudio estuvo constituido por la totalidad de pacientes intervenidos de cirugia renal mayor electiva en nuestra institucion en el periodo antes senalado. Resultados: Estudiamos 571 pacientes, de los cuales 59 (10,33%) recibieron transfusion de hemoderivados. Predomino el sexo femenino y los pacientes de raza blanca con edad promedio de 48,7 anos.  La enfermedad mas frecuente fue  la hipertension arterial (20,8%),  El tabaquismo fue el unico habito toxico analizado en nuestra muestra. Presentamos una tendencia a disminuir la frecuencia de utilizacion de hemoderivados en nuestros pacientes y por tanto el consumo anual de los mismos, el umbral transfusional oscilo entre 22,5 vol% y 25 vol% de hematocrito, las reacciones alergicas fueron las unicas complicaciones observadas en el 5% de nuestra muestra. Conclusiones: El uso de una terapia transfusional restrictiva, asi como la aceptacion de un umbral transfusional bajo nos ha permitido disminuir la  transfusion de sangre homologa en nuestros pacientes quirurgicos renales y con ello la frecuencia de complicaciones transfusionales.  Palabras clave: anemia aguda, hemoterapia, cirugia urologica ABSTRACT Introduction: the current knowledge about the complications associated with the transfusional therapy, the individual adaptability to acute anemia and tisular oxygenation makes the enclosed employment of these therapies more and more restrictive in emergency situations like acute transoperative bleeding. Objectives: to determine the frequency of use and the quantity of transoperative hemoderivates in the major elective renal surgery in our institution, to indicate the transfusional threshold and the tranfusional complications observed. Methods: an observational and descriptive study was realized, of retrospective cohort. The study universe was constituted by the 571 patients who were operated of major elective renal surgery in our institution in the period earlier specified. Results: were studied 571 patients, from whom 59 (10.33 %) received hemoderivates transfusion. Feminine sex predominated and patients of white race with average age of 48.7 years. The most frequent illness was the arterial hypertension (20.8 %), the tabaquism was the only toxic habit analyzed in our study. We present a tendency to diminish the frequency of use of hemoderivates in our patients and therefore the annual consumption of the same ones, the transfusional threshold ranged between 22.5 vol. % and 25 vol. % of hematocrit, the allergic reactions were the only complications observed in 5 % of our sample. Conclusions: the use of a restrictive transfusional therapy, as well as the acceptance of a low transfusional threshold has allowed us to diminish the transfusion of homologous blood in our renal surgical patients and the frequency (5 %) of transfusional complications.
