- W1615722464 abstract "This chapter explains the objectives of various safety analysis reviews. There are many methods, either qualitative or quantitative in nature, of safety analysis reviews that are available and can be applied to a facility or project design to examine and overcome human errors and the various failures of the process system. Quantitative methods, including Checklists, process hazards analysis (PHA), What-If reviews, Hazard and Operability (HAZOP) review and Bow-Tie Analysis (BTA) are usually applied to obtain a more precise evaluation of an identified hazard. These are typically employed for design evaluations and resolution of recommendations when the identified risk is above normally acceptable industry levels and when major capital expenditures need additional justification. Safety reviews primarily look for the possibilities of where human error may occur. The primary objective of PHA, What-If, and HAZOP reviews is to assure that catastrophic incidents will be avoided during the lifetime of the facility from the processes under review. PHA, What-If, and HAZOP reviews are basically a communication exercise, and information is presented, discussed, analyzed, and recorded. HAZOP reviews follow a definitive guide-word (GW) approach, step-by-step. PHA and What-If analyses are usually combined with a checklist to provide a “road map” for the review. PHA, What-If, and HAZOP methods have limitations and advantages, and the chapter presents comparison of these methods." @default.
