- W16270200 abstract "A number of mechanisms have been identified as contributing to protection of the upper gastrointestinal tract. The mucosal surface is covered with a layer of alkaline mucus which has been proposed to provide a first line of defence against acidopeptic digestion by maintaining a near-neutral pH microenvironment at the apical membrane1. Gastric alkaline secretion per unit area of cimetidineinhibited fundus is 2–10% of maximum acid secretory rate and is due principally to Cl−/HCO 3 − exchange. Duodenal alkaline secretion is 3–6-fold higher than in stomach and is due to electrogenic transmucosal transport of HCO 3 − combined with passive permeation of base via shunt pathways2. Using microelectrodes, a stable surface pH gradient can be measured over several hours in both stomach and duodenum3 when luminal pH is 2. In vivo, blood flow provides a source of nutrients, a pool of HCO 3 − , and maintains mucosal acid-base balance4. Gastroduodenal mucosa has a high proliferative rate which ensures replacement of exfoliated cells as a result of mitotic activity. In the shorter term this process is augmented by restitution of epithelial integrity due to migration of surviving cells across the basal lamina to re-epithelialize areas denuded by exposure to, e.g., hypertonic solutions or aspirins." @default.
