- W1628467811 abstract "背景：上顎洞病変に対しての治療は低侵襲性の観点からESSを行う症例が増加しているが部位により操作や観察が困難という面もある。一方で従来のESSと比較しEndoscopic medial maxillectomy（EMM）といった術式は洞内の観察や操作を容易にする。この場合下鼻甲介を含む上顎洞内側壁を切除することから後遺症を引き起こす可能性がある。これらの問題を解決するため鼻涙管と下鼻甲介を温存しつつ上顎洞にアプローチする方法としてEndoscopic modified medial maxillectomy（EMMM）が報告され，そのアプローチを用いた手術の症例数も増加傾向にある。対象：2012年９月より2014年３月までに八尾市立病院耳鼻咽喉科を受診しEMMMを施行した患者11例を対象とした。結果：11例の疾患内分けは副鼻腔嚢胞５例，含歯性嚢胞２例，副鼻腔内反性乳頭腫４例であった。１例に術後出血を認めたが，鼻涙管の閉塞症状や頬部違和感・疼痛，鼻腔乾燥感，鼻閉といった後遺症は１例も認められなかった。全例術後の病変再発は認められなかった。結論：EMMMは広いワーキングスペースのもと大部分の上顎洞内操作を行うことが可能であり，術後の後遺症も非常に少なくメリットの多い術式と考えられた。" @default.
