- W1642675982 abstract "Illiotibial Band Syndrome (ITBS) is the most commonly diagnosed cause of lateral knee pain in runners. With no gold standard in treatment, runners often decrease their training load and intensity to manage recurrence of symptoms. The purpose of this project was to investigate biomechanical associates of injury by studying kinematic differences between runners with and without ITBS. Twenty-seven adults (19 females, 8 males; 31.25±16.25 yrs) were assigned to two groups: ITBS (n=12) or control (n=15). Participation included a clinical screen for the presence of ITBS by a licensed Physical Therapist, and gait analysis. Three-dimensional hip kinematics were recorded utilizing a three camera Vicon Bonita system (200 Hz) while subjects ran on a treadmill. Temporal running data were normalized as a percentage of the stance phase (%Stance). Three separate one-way (group) ANOVAs were used to assess differences between the injured leg in ITBS and the dominant leg of control runners for: the timing of peak hip adduction angle (%Stance) and mean peak stance phase hip adduction angle (deg) and velocity (deg/sec). Runners with ITBS reached peak hip adduction sooner in the stance phase compared to controls (ITBS: 36.33%±17.89%, Control: 56.47%±27.69%, P = 0.04). Mean peak hip adduction angle and velocity was not different between groups (ITBS: 8.15°±3.53°, control: 8.79°±3.81°, P = 0.69 ; ITBS: 29.68°/s±13.19°/s, control: 33.79°/s±24.38°/s, P =0.61, respectively). The timing of peak hip adduction angle between groups was significantly different. This suggests that runners with ITBS demonstrate an early offloading of the injured limb during the weight acceptance phase of gait, indicating the possible presence of an anticipatory pain avoidance strategy. Although it has been suggested that ITBS strain rate is a contributing factor to injury, the lack of differences between groups in magnitude of peak hip adduction angle and velocity suggests otherwise." @default.
