Further investigations along these lines should provide new insights into the intrinsic mechanisms of pain modulation. The mechanisms involved in peripheral opioid analgesia are discussed in the chapter according to the schematic drawing that illustrates the events at the terminal region of a nociceptive nerve fibre at various developmental stages of the inflammatory process. Plane1 A represents the normal condition in the absence of inflammation or tissue injury. There is general agreement that in this case peripheral sites do not play a significant role in the antinociceptive effects of opioids. This is remarkable in view of the fact that opioid receptors (binding sites) can be demonstrated at the primary afferents of somatosensory fibres of the non-inflamed tissue. It has to be concluded that these receptors are non-functional under such conditions." @default.
