- W167549465 abstract "250 Objectives To assess the value of C-11 Metomidate PET/CT in patients with clinically suspected PHA Methods C-11 Metomidate PET/CT was used to assess adrenal function. Ratio of the standard uptake value (SUV max) between both adrenal glands were calculated. The final diagnosis was based on histopathology, and clinical follow- up. Results 32 patients were identified, mean age 53 and follow up for 20 months. 14/32 patients demonstrated very high focal C-11 metomidate uptake in the adrenal gland with a ratio 1.8 In 13/14 patients the uptake of tracer was associated with adrenal nodule (mean 1.6 cm) but in 1/14 patient the adrenal glands had normal appearance on CT. 11/13 with adrenal nodule underwent surgery and histology confirmed presence of adrenal adenoma, and clinically improved after surgery. 2/13 patients opted for conservative treatment, they were clinically proven to have PHA.. 11/32 patients showed moderate tracer uptake in the adrenal gland with a ratio between 1.0 and 1.24 . In 6/11 patients with increased tracer uptake were associated with adrenal nodule (mean 0.8 cm). 2/6 patients underwent surgery based on matching AVS lateralization and histology findings confirmed adrenal adenoma, and clinically improved after surgery. In remaining 5/11 patients no adrenal lesion was seen on CT. None of the 9 patients who did not underwent surgery, proved to have PHA on further clinical follow up. In 5/32 patients, hypodense lesions with low tracer uptake (ratio 0.7) were seen on PET/CT consistent with non-secreting tumour confirmed by clinical follow-up. 2/32 with multiple adenomata with heterogeneous tracer uptake (ratio 1.45) showed equivocal results on clinical follow up. Overall sensitivity was 87 %, specificity was 81%, PPV was 83 % and NPV was 86 % Conclusions C-11 Metomidate PET/CT is evolving to be an useful imaging modality to assess and managing patients with PHA." @default.
