- W1678289341 abstract "This chapter reviews current evidence concerning the extent to which intakes of calcium and vitamin D nutrients influence bone metabolism, describes a rapid approach to estimating intake, and suggests approaches to help individual patients meet the recent intake recommendations. Calcium and vitamin D intakes alter calcium regulating hormone levels and bone mass. An inadequate intake of either of these nutrients results in a reduced amount of absorbed calcium and subtle lowering of the circulating ionized calcium concentration. The latter then stimulates the release and raises the blood level of parathyroid hormone (PTH). The PTH returns the blood calcium level to normal by decreasing the renal excretion of calcium, by promoting the activation of vitamin D (which stimulates calcium absorption), and by promoting bone resorption. In older people, bone remodeling is uncoupled in favor of bone resorption. Because of this, the PTH-induced increase in bone remodeling in such individuals results in an increased rate of bone loss. Several approaches can be taken to increase calcium and vitamin D intakes to recommended levels. It is preferable to meet the calcium and vitamin D intake requirements by adjusting the diet. For those who cannot or will not consume the needed amounts of these nutrients from natural and fortified foods, supplements may be needed. When using supplements, several issues must be considered including the choice of a specific calcium salt, the dosage and schedule, and the safety of the supplement." @default.
