- W169088397 abstract "For women in the USA, the mortality rate of breast cancer is second only to that of lung cancer and has remained essentially unchanged for the past 60 years, if not longer (Seidman and Mushinski 1983). Earlier detection by means of screening with physical examination and mammography can significantly improve on this (Shapiro et al. 1985; Habbema et al. 1986; Baker 1982; Tabar et al. 1985; Verbeek et al. 1984; Collette et al. 1984; Anonymous 1984; Letton and Mason 1986; Seidman et al. 1987). Despite this fact, and despite the longstanding, vigorously advocated guidelines of the American College of Radiology (ACR; McLelland and Feig 1983), the American Cancer Society (ACS; ACS 1983) and other medical organizations, surveys and anecdotal experiences reveal that screening mammography is being done only in a small percentage of eligible women (Bassett et al. 1985; Anonymous 1985; Fox et al. 1985). There are a number of explanations for this noncompliance, including: (1) uninformed physicians and patients; (2) skepticism regarding cost-effectiveness, especially because of the low yield of all screening procedures; (3) confusion because of changes in the guidelines for mammography (Moskowitz 1986) and variation in them by different medical organizations; (4) misconceptions regarding radiation risks, which are clearly hypothetical and minimal, if not negligible, especially when compared with the proven benefits (NCRP 1986; Feig 1986); (5) less than optimal results, even with symptomatic patients, when examinations are carried out by insufficiently trained, inexperienced radiologists (Martin et al. 1979) and also other physicians (Fletcher et al. 1985); and (6) the charge for screening, especially mammography." @default.
