• W1697432876 abstract "Die erfolgreiche Transplantation von Schweineorganen auf den Menschen, also die Xenotransplantation, ist angesichts der zunehmenden Knappheit von Allotransplantaten ein vorrangiges Ziel der modernen Transplantationsforschung. Zur Untersuchung dieser Moglichkeit wurden deshalb bereits viele experimentelle Studien mit Primaten als Empfanger durchgefuhrt. Neben medikamentoser Immunsuppression, Komplementhemmung und Toleranzinduktion ist die Entfernung xenoreaktiver Antikorper bei der Schwein-zu-Primat-Xenotransplantation von entscheidender Bedeutung.In der vorliegenden Studie untersuchten wir die Anwendbarkeit der Ig-Therasorb®-Immunadsorption zur extrakorporalen Elimination xenoreaktiver Antikorper in einem Schwein-zu-Primat-Modell, sowie die technische Durchfuhrung, die Nebenwirkungen und die Effizienz der Behandlungen. Die Immunadsorption wurde dabei zur Vermeidung und Therapie der hyperakuten und akuten vaskularen Xenotransplantatabstosung eingesetzt. Erstmals wurden auch hDAF-transgene Schweineherzen in Kombination mit Ig-Therasorb®-Immunadsorption verwendet, um eine zusatzliche Hemmung des Komplementsystems zu erreichen. Wir fuhrten 2 Versuchsreihen mit 7 Cynomolgus- und Rhesusaffen sowie 4 Pavianen als Empfanger zur Evaluierung der Technik und Effizienz der Immunadsorption durch sowie 4 Versuchsreihen mit insgesamt 18 Pavianen zum Langzeituberleben nach orthotoper und heterotoper Transplantation von Landrasse- und hDAF-transgenen Schweineherzen mit perioperativer Immunadsorption.Die Ig-Therasorb®-Immunadsorption ist eine effiziente und sichere extrakorporale Plasmaperfusionsmethode zur Entfernung xenoreaktiver Antikorper. Eine hyperakute Abstosung kann durch die praoperative Behandlung mit Immunadsorption vermieden werden, wenngleich noch untersucht werden muss, ob die Immunadsorption auch bei der Vermeidung einer akuten vaskularen Abstosung eine verlassliche Methode ist. Es konnte zudem bestatigt werden, dass hDAF-transgene Schweineherzen einen Schutz vor der hyperakuten Xenotransplantatabstosung bieten. Die Ig-Therasorb®-Immunadsorption hat sich bereits in vielen Studien mit Patienten als sicher und effizient erwiesen, auch in der langfristigen Anwendung. Es ist zu erwarten, dass die Methode bei einer zukunftigen klinischen Xenotransplantation von den Patienten gut vertragen wurde und einen entscheidenden Beitrag zur Vermeidung einer HAR und zur Verlangerung der Transplantatuberlebenszeit leisten konnte." @default.
