- W1738372504 abstract "Post-stroke painful shoulder and central pain are important causes of incapacity that interfere with the efforts that the patient must make during rehabilitation. Shoulder pain is a common syndrome in persons who suffer a cerebrovascular accident (CVA). The principal etiological factors are muscular imbalance, induced by periarticular muscle weakness and spasticity of the internal abductor and rotator muscles, together with inadequate maneuvers suffered by hemiplegic patients who depend on the aid of other persons for their mobility. What is most important is to take an attitude of prevention: postural treatment that prevents muscular and tendinous shortening; the avoidance of microtraumatisms on the paretic shoulder; giving support to the limbs to prevent stretching of the soft tissues and subluxation and carrying out the physiotherapy adequately. When pain appears in the shoulder, an attempt should be made to reach an etiological diagnosis as is done in the general population without CVA, it being possible to use the same medical and therapeutic interventions. The central pain generally occurs months after the CVA and its frequency of presentation is greater than that which has been expected. It is important to identify it and treat it. It is manifested by dysesthesic disorders and can be related with allodynia and hyperpathy, in all the hemibody or in a more localized region.
