- W1750577071 abstract "Although eczema is most common in infancy, epidemiologic studies have focused on older children. We analyzed international differences in eczema prevalence and associated factors at its peak prevalence age. We determined prevalence rates of parent-reported eczema in the first year of life, and to analyse the association with putative risk and protective factors, in two European and two Central American countries. A standardized, validated questionnaire was used to assess parent-reported eczema and factors for which associations with eczema have previously been reported in older children. Data from 9803 infants in the International Study of Wheezing in Infants (EISL) were analysed. Odds ratios (OR) with 95% confidence intervals were assessed by logistic regression. Eczema prevalence varied from 10 . 6% (Valencia, Spain) to 28 . 2% (Honduras), and was significantly lower in Europe (14 . 2%) than in Central America (18 . 2%, p . 0001). Consistent with older children, presence of siblings decreased (OR 0 . 819 [0 . 717 – 0 . 936]), whereas family history of asthma (OR 1 . 322 [1 . 100 – 1 . 587]), rhinitis (OR 1 . 325 [1 . 142 – 1 . 537]) and eczema (OR 2 . 404 [1 . 892 – 3 . 054]) increased eczema risk. Eczema was more common in infants who experienced wheezing (OR 1 . 831 [1 . 482 – 2 . 264]). Gender, family size, ethnicity, breast feeding and socio-economic status were not associated with eczema prevalence. Conclusions – Infantile eczema and wheeze likely are early co-manifestations of the atopic phenotype. Factors influencing the expression of eczema in older children only partly apply to infants, possibly suggesting a different pathophysiology of eczema at its peak prevalence age." @default.
