- W1750692711 abstract "To assess the performance of EA at the time of SIS in evaluation of endometrial pathology in postmenopausal women. Retrospective study of 477 postmenopausal women age > 50 who underwent saline-infusion sonohysterography (SIS) for evaluation of postmenopausal bleeding or other increased risk for endometrial pathology. For all patients, at the end of imaging, the remaining intra-cavitary saline was aspirated through the SIS catheter and sent for histological evaluation. Patients were then managed by their providers with endometrial sampling, hysteroscopy, hysterectomy or observation. For all patients who had formal histological evaluation, the sensitivity, specificity, PPV and NPV of EA were calculated. Endometrial tissue was identified in 451/477 (94.5%) EA specimens. 280/477 had a formal histological evaluation following the SIS in a separate procedure. Of them, 21 patients (7.5%) were diagnosed with either endometrial cancer or hyperplasia. All 21 cases had abnormal findings on their SIS. In 17 of the 21 cases, the EA results correlated with final pathology providing a sensitivity of 80.9%, specificity of 100%, PPV of 100% and NPV of 98.4%. In contrast, in the 188 cases (39.7%) in whom a polypoid structure was imaged, EA agreed with this finding in only 24 cases (12.7%). EA at the time of SIS is highly specific for evaluation of endometrial hyperplasia and cancer in postmenopausal women. This simple step should be considered as an integral part of the procedure and may allow elimination of a separate endometrial sampling procedure prior to the planning of definitive surgery in the subset of patients with abnormal results." @default.
