• W175890305 abstract "We sought to explore the relationship between C4d biopsy status and DSA levels. We compared DSA mean fluorescent intensity (MFI) levels in 75 renal transplant biopsies, including 25 cases that were C4d positive (>50% peritubular capillaries stained (PTC)), 25 cases that were negative (<20% stained PTC) and 25 that were equivocal (20–50% PTC stained). All DSA MFI levels were determined by single antigen Luminex® beads, and all cases were from a single transplant center between 2010–2012 with DSA levels obtained within 0–4 days of the corresponding biopsy. A significant difference was shown between positive C4d Class I DSA and equivocal C4d Class I DSA. Due to MFI variability in the Class II DSA, there was no difference between equivocal and positive C4d. Eighty percent of negative C4d had no DSA or low DSA (<1500 MFI). In contrast, the equivocal C4d cases had variable DSA results. All positive C4d DSA MFIs were significantly different compared to negative DSA MFIs. Equivocal C4d cases that had Class II DSA were not significantly different from Class II DSA found in positive C4d patients. We postulate that Class II DSA is not expressed well in the PTC and the equivocal C4d occurs only with Class II DSA.
