- W17605053 abstract "Although rheumatoid arthritis (RA) is primarily an autoimmune disease which affects the synovial joints, it has profound effects on bone growth and remodeling. This chapter reviews the normal processes involved in bone remodeling, the mechanisms by which they are controlled, and highlights the changes induced in RA that have a high impact on those processes. Bone changes are the result of alterations in paracrine regulation of remodeling and do not depend on the direct action of exogenously produced enzymes. The expansion of knowledge of the control of remodeling now suggests that causes for the changes in RA are due primarily to eicosanoid and cytokine mediated influences on events of the bone remodeling cycle. Increased activation or decreased formation during the process, both of which suggest modification of the coupling process, could result in bone loss. Bone remodeling is profoundly influenced by the actions of many cytokines, including those specifically affected by inflammatory responses. Inhibition of the osteoclast proton pump, PGHS-2 synthase, and cytokine synthesis are examples of logical targets for therapeutic intervention in pathological bone loss. Both formation and resorption are elevated in RA, even during the acute phases of the disease, suggesting that intervention in the control of remodeling may occur at multiple sites in the process." @default.
