- W1769294013 abstract "OBJECTIVE:To describe a case series of 6 patients with Pseudotumor Cerebri (PC) who received venus sinus stenting (VSS).BACKGROUND: PC is a disease with significant morbidity. Medical management includes weight loss and carbonic-anhydrase inhibitors. Accepted surgical therapies for medically refractory PC include Lumboperitoneal shunt, ventriculoperitoneal shunt and optic nerve sheath fenestration. In a subset of patients with PC and venous sinus stenosis, VSS has emerged as an alternative surgical approach.DESIGN/METHODS:We describe 6 patients with medically refractory PC who were treated at 3 tertiary care facilities. All patients underwent diagnostic venogram with pressure measurement across the cerebral venous drainage. Once a pressure gradient was identified, the side with the higher gradient was selected for stenting.RESULTS:8 types of stents were placed (4 peripheral vascular, 2 biliary, 1 coronary and 1 carotid stents). 5 stents were self-expandable and 3 were balloon expandable. Attempts to place biliary stents (Xpert and Visio-Pro) were unsuccessful. The placement of balloon expandable coronary (Veriflex) and renal (Herculink Elite) stents was facilitated by balloon angioplasty (BA) of proximal internal jugular vein. The Precise carotid stent needed BA as well. 3 patients received Complete self-expanding vascular stent that required no BA. In one patient, the stent jumped forward and missed the site of stenosis and required re-stenting. All patients had complete resolution of headaches and no peri-procedural complications.CONCLUSIONS:Due to larger diameter of venous sinuses, traditional intracranial stents cannot be used for VSS. Different types of peripheral stents have been used with variable success. Various technical problems include difficulty in stent delivery to the site of stenosis, failure to deploy stent at the right place of stenosis and residual stenosis requiring post stent angioplasty.
