The knowledge of normal adrenal weight is essential for the correct interpretation of surgical adrenalectomy specimens, particularly from patients with Cushing's disease. The generally quoted normal weight, 6-7 grams per adrenal, is derived from routine hospital autopsy specimens and is not applicable to surgical material. Patients who die in hospital have usually experienced severe physiological stress terminally, resulting in a degree of hyperplasia. In patients who have died acutely of trauma, the average adrenal weight is 4–4.5 g , whereas in patients who have had adrenalectomy in the treatment of cancer of the breast, the average weight is about 3.5 grams. The stimulation of the adrenal cortex by ACTH, which may occur acutely as with the stress of any severe disease or chronically with either an exogenous or endogenous source, produces pathological changes that are striking. The zona fasciculata shows initially focal and later generalized, lipid depletion and replacement of the clear cells in its columns by eosinophilic compact cells larger than those of the normal zona reticularis. With severe change from ACTH stimulation, the normal zonal differentiation of the cortex is lost." @default.
