Abstract Dental caries is a highly prevalent disease worldwide and WHO consider it as major Public Health problem that affects all ages. Along with adequate preventive actions, caries diag- nosis in its initial phase, before cavity formation, assumes a big role in controlling the disease. The objective of this paper is to review the etiological factors of the disease, describe what's going on dental hard structure during the carious process, before cavity, and the importance of diagnosing dental caries in this early stage. The authors discuss the demineralization process that runs along with the disease by doing a bibliographic review in Medline. In few years, new dental caries paradigms were held in result of the understanding of the physiopathology and the cavitation process. For example, the term lesion is being part of it. It is also reviewed a specific mani- festation of the disease, early childhood caries, highlighting the early diagnosis in a way that allows to act in a phase where cavity is not yet present. Conclusion: due to the actual understanding, dental caries can be better diagnosed in earlier stages. This issue can result in preventing cavity formation and easier control of the dis- ease. Paediatricians can play a major role in referring, to the dental office, patients with dental caries precavitated lesions." @default.
