The basic maneuvers of head elevation, sedation or pharmacologic paralysis, hyperventilation, cerebrospinal fluid drainage, and osmotic therapy remain the fundamentals of treatment. Barbiturates appear to have a role in treating a select group of patients with refractory intracranial hypertension. There is increasing evidence, however, that we may soon be able to differentiate distinct subgroups of patients with raised intracranial pressure (ICP) who should be treated in differing fashions. Classifying patients by age, computed tomography diagnosis, responsiveness to hyperventilation, pressure-volume index, and ICP waveform may allow us to select specific treatment strategies in individual patients and to target some patient groups for altered protocols or new therapeutic agents." @default.
