• W1821996997 abstract "Die Resistenz von Blasten der akuten myeloischen Leukamie kann auf verschiedenen Mechanismen beruhen, zu denen die Expression von P-Glykoprotein (Pgp) gehort, einem Membranprotein, das Anthrazykline, Vinkaalkaloide, Taxane und andere Xenobiotika energieabhangig aus der Zelle transportiert.In der vorliegenden Arbeit wurde das Phenylpteridinderivat BIBW22BS als potentieller Inhibitor der Pgp-vermittelten Multidrugresistenz (MDR) bei Blasten, die aus dem peripheren Blut von Patienten mit erstdiagnostizierter und rezidivierter/refraktarer akuter myeloischer Leukamie gewonnen wurden, ex vivo untersucht. Auserdem wurde der Einflus von BIBW22BS auf die Zytotoxizitat von Cytosinarabinosid (Ara-C) in einer sensiblen und einer resistenten Zellinie in vitro untersucht.Dabei ergaben sich folgende Befunde:1.BIBW22BS fuhrte bei den peripheren Blasten ex vivo zu einer signifikanten Hemmung des Efflux von Rhodamin 123 und dem Anthrazyklin Daunorubicin.2.Der Ex vivo-Efflux des Anthrazyklins Idarubicin aus den peripheren Blasten wurde im Gegensatz dazu nicht von BIBW22BS beeinflust.3.Die Inkubation der sensiblen Pgp-negativen Zellen mit Ara-C in vitro ergab eine dosis- und inkubationszeitabhangige Zytotoxizitat.4.Die Zytotoxizitat von Ara-C in den sensiblen Pgp-negativen Zellen konnte in vitro durch Koinkubation mit BIBW22BS zu 80 bis 100% aufgehoben werden.5.BIBW22BS hatte bei den resistenten Pgp-positiven Zellen in vitro keinen Einflus auf die insgesamt geringere Zytotoxizitat von Ara-C.Insgesamt erwies sich damit BIBW22BS als Inhibitor des Pgp-vermittelten Efflux von Anthrazylinen. Gleichzeitig wurde aber die Zytotoxizitat von Ara-C durch BIBW22BS in den Pgp-positiven Zellinien gehemmt, was moglicherweise uber eine Wechselwirkung mit dem Nukleosidtransporter zu erklaren ist.Damit wird der positive Effekt von BIBW22BS auf die MDR moglicherweise durch die Hemmung der Zytotoxizitat von Ara-C bei der Behandlung der AML aufgehoben, so das der klinische Einsatz von BIBW22BS zur Verbesserung der Therapie und Uberwindung der MDR kritisch beurteilt werden mus." @default.
