- W1839537104 abstract "Surveillance of MRSA infections is not routinely performed in outpatient clinics; however, our microbiology laboratory has been reporting positive MRSA cultures from inpatient and outpatient settings to Infection Control since 2010. This practice allowed us to identify four patients who developed MRSA wound infections over a three month period after undergoing outpatient procedures in a Dermatology Clinic. A case investigation was conducted by Infection Control to evaluate the unexpected high rate of MRSA infections post outpatient dermatologic procedures. Charts of all infected patients were reviewed. Infection Control met with the clinic staff, observed different procedures, gave impromptu in-services and assessed the environment. Attempts were made to retrieve the MRSA isolates from the microbiology laboratory to determine relatedness. From the findings of the investigation, Infection Control was able to recommend best practices for the Dermatology Clinic in order to decrease their infection rate. During a three month period, 757 outpatient procedures were performed in the dermatology clinic and four (0.5%) resulted in MRSA wound infections an average of 13 days post procedure (range 12-14 days). A chart review of the four patients revealed that none were known to be colonized or have past infections with MRSA. Three of the four procedures were performed by the same provider. The MRSA isolates were not saved and therefore no further testing could be perfomed. Multiple factors may have contributed to an increase in MRSA infections complicating outpatient dermatologic procedures. Infection Control recommended several of the Association of the Operating Room Nurses Perioperative Standards and Recommended Practices and there have been no further post procedure infections over three months.
