Patients with chronic renal failure have been maintained on hemodialysis for over 20 yr. Ironically, this extension of life has permitted the clinician to recognize the many shortcomings of hemodialysis therapy. Patients on hemodialysis continue to manifest metabolic derangements that include hyperlipidemia, anemia, coagulation disorders, osteodystrophy, neuropathy, and sexual dysfunction. With the current state of the art, hemodialysis is but a crude approximation of the normal kidney’s excretory and acid-base function, its endocrine and metabolic functions being beyond the scope of hemodialysis. For more than a century, attempts have been made to correlate the clinical manifestations of uremia with the retention of specific uremic toxins. These attempts have been singularly unsuccessful.
