- W186988051 abstract "Publisher Summary This report deals with some of the practical utilizations in neurosurgery of the intracarotid and intravertebral injection of drugs. The direct injection of drugs into one of the main arteries supplying the encephalon has been largely utilized in experimental neurophysiology. The identification of the side of speech representation is a relevant problem in neurosurgery. Its importance is particularly evident when one has to operate on the temporal lobe in absence of gross brain pathology, as for instance in temporal epileptic patients. It is well known that handedness is not necessarily related to the side of speech dominance. The effect produced by the intracarotid barbiturate on the epileptic discharges of the ipsilateral and of the contralateral hemisphere in patients with bilateral non-synchronous epileptic activity can be different in the different subjects. It is likely that these differences are to a great extent related to the spatial location of the epileptogenic zones and to the reciprocal influences between the epileptogenic zones of the two hemispheres. The aim of the present report is that of providing evidence for the practical utilization of the intracarotid and intravertebral injection of drugs. The few examples reported were sufficient to illustrate the possible use of the method in clinical fields of neurosurgical interest. The method can be safely employed and the use of other drugs, besides the ones so far utilized, may provide in the future even more information." @default.
