- W18996132 abstract "This study examines the relationship between maternal reflective functioning (RF), stress and depression in mothers with preterm infants in the Neonatal Intensive Care Unit (NICU). RF refers to the ability to think about mental states and link behaviour to underlying mental states in oneself and others. Previous research indicates that the NICU environment can cause stress and depression, which may impact on the development of a secure infant-mother relationship. It has been suggested that maternal RF may be critical in predicting a mother’s capacity to respond sensitive and appropriately to her child (Fonagy et al., 1991) and RF has been linked to the quality of parent-child relationship (e.g. Patterson, 2005) and infant’s attachment security (Fonagy et al., 1995; Slade et al., 2005). Recently, RF has emerged as a protective factor for the infant-parent relationship in situations where the family is experiencing high stress levels (Patterson et al., 2005; Fonagy et al, 1995). It was hypothesized that mothers with high RF ability would report lower levels of stress and depression during their child’s NICU admission, as compared with mothers demonstrating low RF capacity. Twenty-five mothers, with 7- to 14-day-old preterm infants admitted in the Royal Brisbane Women’s Hospital, were interviewed using the Parent Development Interview (PDI) (Slade et al., 2005). The Reflective Functioning Scoring Manual was used to score maternal RF on the PDI. The Parental Stress Scale: NICU and the Edinburgh Postnatal Depression Scale were administered to measure maternal stress and depression, respectively. Preliminary results indicated that mothers who demonstrated high RF capacity were significantly more likely to be classified as not depressed (92%) than depressed (8%). Mothers with low RF capacity were equally likely to be depressed as not depressed. This relationship was evident after controlling for previous abuse/trauma experienced by the parent. No significant relationship was detected between RF and stress. This may be due to the small sample size and lack of power, or it may be that RF serves as a protective factor for stress only in situations involving extreme stress (e.g. deprivation, abuse), and the NICU environment may not be perceived as extremely stressful overall. This study takes the first step in understanding the relationship between stress, depression and RF in a preterm infant-parent sample. Data collection continues for the 7-14 days post-birth time point, and this sample will be followed up at 6 months corrected age to assess infant-parent interaction quality. This will increase our understanding of how maternal stress, depression and RF shortly after birth interact to predict parent-infant relationship quality in the first year." @default.
