- W1917222214 abstract "Gunumuzde cerrahi hastalarinin perioperatif donemde basarili idaresinin, sivi elektrolit dengesinin korunmasina bagli oldugu bilinmektedir. Bu nedenle kan, sivi ve elektrolit degisikliklerinin dogru olarak degerlendirilmesi ve eksikliklerinin dogru olarak yerine konmasi cok onemlidir. Ancak, cerrahi prosedurlerin neden oldugu bu akut sivi elektrolit degisikliklerinin nedeni tam olarak bilinmediginden gerekli onlemlerin alinmasi da guclesmektedir. Farmakokinetikleri ve farmakodinamikleri farkli olan sivilar replasman amaci ile yaygin olarak kullanilmaktadir. Kardiyak cerrahi hastalarinda sivi rejimi secimi yapilirken sivilarin yalnizca sistemik hemodinamik etkileri degil ayni zamanda mikrosirkulasyon, pulmoner fonksiyonlar, eritrosit reolojisi ve koagulasyon sistemi uzerine olan etkileri de dikkate alinmalidir.Kalp cerrahisi uygulanacak hastalarda ideal volum tedavisinin saglanabilmesi icin surec, KPB oncesi donem, KPB’in baslangic volumu, KPB sonrasi ve YBU donemi olarak 3 periyotta incelenmelidir. Kalp cerrahisi hastalarinin KPB oncesi sivi tedavisi ile ilgili birkac calisma olup, sonuclari birbirinden oldukca uzaktir. Bu calismalar yalnizca kristaloid kolloid karsilastirmasi seklinde degil, ayni zamanda hangi kolloid sorusuna da yanit verecek sekildedir.Kristaloid kullananlar calismalarda kristaloidleri ustun bulmakta, kolloid kullananlarda calismalarinda kolloidleri ustun bulmaktadirlar. Bu nedenle de bugun icin net bir yanit bulmak olanaksizdir. Yalnizca bugun icin net olarak ifade edilebilecek olan, Amerikan Yiyecek ve Ilac Birligi’nin (FDA) daha once kullanimina izin verdigi birinci kusak HES (MA>450 kD, MS>0,7 tuzdaki solusyonlarinin) solusyonlarinin, KPB sonrasi kanamayi artirmasi nedeni ile, ozel bir durum olan KPB uygulamalarinda kullanimini onermemesidir." @default.
