Zur Therapie sollte ein Antimykotikum eingesetzt werden, das möglichst geringe Nebenerscheinungen zeigt, sich gegenüber Resistenzentwicklungen neutral verhält, gut verträglich und einfach in der Handhabung ist. Dies gilt in besonderem Maße für die Langzeitbehandlung. Im vorliegenden Fall konnte eine schwere Augenmykose mit Miconazol i. v. - dem ersten parenteral verfügbaren Antimykotikum aus der Reihe der Imidazolderivate - erfolgreich therapiert werden. Die Behandlungsdauer betrug 92 Tage. Nebenwirkungen traten selbst unter der gesamten Dosis von 62,0 g Miconazol i. v. nicht auf. Summary: The treatment of mycoses of internal organs is handicapped by the uncertainty of the clinical diagnosis, the problems of laboratory diagnostics and a therapeutical situation, which is still unsatisfactory. The use of available antimycotics for the treatment of organic and systemic mycoses requires judicious weighing up of the clinical picture, diagnostic data and therapeutic risk. For therapeutic purposes an antimycotic should be used, which is low in side-effect liability, neutral as regards the development of resistance, well tolerated and easy to handle. This applies in particular to long-term treatment. In the case under review a severe mycosis of the eye was successfully treated with intravenous miconazole - the first antifungal agent of the group of imidazole derivatives for parenteral administration. The duration of treatment was 92 days. Side-effects did not occur, even a with total dosage of 62,0 g of miconazole i. v.
