- W1962407526 abstract "Generosity and profound care for each individual patient are a characteristic of many of the doctors and health care professionals I met from the Diabetes Association of Bangladesh (DAB) and the Bangladesh Institute of Research and Rehabilitation in Diabetes and Endocrine Metabolic disorders (BIRDEM). As is true in many developing countries, the health care sector struggles to obtain the necessary medications, trained physicians and professionals, and to offer as much financial assistance to its patients as possible. The Government in Bangladesh has many problems on which to focus, such as the prevention of the spread of waterborne diseases, malaria, diarrhoea and family planning. When it comes to conditions such as diabetes, which affects thousands of potentially treatable patients, the Government has little money to spare. Therefore the DAB has largely had to fend for itself, and out of its struggles has come an efficient, well directed and structured organisation, testament to what is possible, even in the direst of conditions. The DAB endeavours to overcome many challenges, explained Professor Azad Kahn, Secretary General of the DAB, such as a significant lack of resources, the economic situation of most of their patients, and a low literacy rate. In many ways the DAB has been able to alleviate at least some of these problems. Compared to the previous two countries I visited, El Salvador and Tanzania, Bangladesh has a strong central diabetes association. The dedicated health care workers struggle to assist and treat the sheer masses of people in need of care. The BIRDEM hospital is only in the capital city, though it is attempting to establish clinics in more remote locations. The scarcity of trained doctors has made diabetes education one of the DAB's top priorities. There is hope for patients, especially in the long run, due to the efforts of the DAB. During my visit, the first 50 doctors to be trained in a six-month course, designed to educate general practitioners on diabetes and methods for treating patients, were holding one of their meetings in the BIRDEM general hospital. In the past Novo Nordisk has helped BIRDEM obtain more insulin, by donating an additional 11% of whatever is purchased to the hospital for use by the poorest patients. Additionally, the company is now lowering the cost of insulin to 20% of the standard price it is sold at in Europe, making it more available to those with modest means. In all three countries, one of the greatest hopes of the people with diabetes whom I met was for an end to bearing the burden of diabetes alone. Confronting the challenges presented by the developing world will require collaboration on many fronts. Yet if my journey around the world has taught me one thing, it is that sustainable solutions to these challenges are possible, feasible, and long overdue." @default.
