- W1963508688 abstract "Summary The value of epidemiological studies in research on ischaemic heart disease is irrefutable. The approach is either retrospective or prospective. The former approach assumes that any differences found between patients and controls exist at the onset of the disease. The latter approach requires frequent examinations of a large population group before the event, in order to determine the factors accelerating its onset. Both approaches have their limitations but are important and interdependent. Recent retrospective studies indicate a switch in emphasis from cholesterol to triglyceride metabolism as the error in ischaemic heart disease. This error may arise from mechanisms in the upper gastro-intestinal tract. In the evolution of ischaemic heart disease there appears to be an interplay of both genetic and environmental factors. Even amongst the known and measurable environmental factors, inter-relationships can be shown to exist. All these factors may operate in differing degrees in different communities. Until prospective studies prove otherwise, the best explanation at present is that the many different clinical syndromes comprising ischaemic heart disease are founded on a multi-factorial aetiology." @default.
