- W1963669925 abstract "Objective: To determine if serum leptin levels are predictive of outcome of in vitro fertilization. Methods: A nested case-control study was performed on patients undergoing in vitro fertilization (IVF) at a university-based fertility center. All patients received a GnRH agonist prior to stimulation then were administered urinary gonadotropins according to a set protocol. Human chorionic gonadotropin (hCG) was administered when follicles reached maximum maturity by sonographic criteria. At the conclusion of the stimulation cycle, subjects were divided into two groups (low and high responders) based on the number of follicles seen by ultrasound on the day of hCG administration. The low responder group was defined as subjects with less than 12 follicles measuring ≥13 mm on the day of hCG. High responders were subjects with ≥12 follicles measuring ≥13 mm on the day of hCG. Subjects had blood drawn on the morning that stimulation was started and on the morning that hCG was administered. The serum was stored at −70°C until assayed. Leptin was measured using a commercially available EIA and estradiol was assayed using the Abbot TdX system. The mean leptin and estradiol levels were compared between the two groups using student’s t test and Pearson correlation coefficients were calculated. Results: A total of 37 subjects were included. There were 13 low responders and 27 high responders. No significant demographic differences were detected between groups. As anticipated, the low responders had significantly lower peak estradiol levels (mean ± SEM) (low responder = 1776 ± 146 pg/ml, high responder 3534 ± 148 pg/ml, p<.05). Mean leptin levels at the start of the cycle were 21.5 ± 2.8 ng/dl for the low responders and 34.6 ± 4.4 ng/dl for the high responders; on the day of HCG leptin levels were 53.2 ± 5.4 ng/dl for the low responders and 50.4 ± 2.95 ng/dl for the high responders. The differences between the groups were not significantly different. Leptin correlated directly with body mass index (r = .77, p<.05). There was no correlation between leptin and age, number of follicles retrieved, total number of ampules of gonadotropin used, days of stimulation, embryo quality or estradiol levels. Conclusions: As demonstrated by Butzow and colleagues, (JCEM 1999;84:3135) leptin levels increased significantly during the IVF stimulation cycle. However, contrary to Butzow’s data, we demonstrated that leptin levels did not correlate with the number of follicles observed or with the number of oocytes retrieved. Mean leptin levels were not significantly different between low and high responders. Therefore, leptin levels at the start of stimulation or at the time of hCG administration do not appear to be predictive of IVF outcome." @default.
