- W1963670042 abstract "Vascular endothelial growth factor (VEGF) plays a known role in regulating the tone vasomotor and maintaining blood pressure (BP). Tao and coworkers1 describe in a recent article the role of VEGF in the pathophysiology of hypertension and possible future therapeutic implications. Several studies have demonstrated the role of VEGF and endothelial viability factors. Endothelial integrity and viability is crucial in many situations, including interischemia-reperfusion, neoplasia, inflammation, perfusion of organs for transplantation, shock, and re-endothelialization of denuded surfaces.2 Endothelial dysfunction plays a major role in the development and maintenance and complications of hypertension and is a fundamental concept that explains the mechanisms by which hypertension is involved in the progression of atherosclerosis. On the other hand, nitric oxide (NO) is considered the most important endogenous vasodilator body and its deficiency is associated with elevations in BP. VEGF is a potent signaling molecule whose function is to stimulate endothelial NO synthase (eNOS) for causing release of NO vasodilatation.3 The pathways inhibition of VEGF-dependent vasodilation, such as NO, can cause capillary rarefaction with a consequent increase in tubular reabsorption of sodium and vascular resistance. The speed with which patients develop hypertension suggests that inhibition of NO plays a primary pathophysiological role. When blocks binding of VEGF to its receptor activity are inactivated, the enzyme eNOS causes reduction in NO production and increased vasoconstriction.4 For this reason, we make an effort to monitor the effects of hypertension and kidney on different VEGF inhibitors in our patients.5 However, we must not forgot that VEGF is produced by different tumors, and its production is related with aggressiveness, potential for spread, and recurrence. VEGF-A is considered the primary tumor angiogenic factor, acting together with other related molecules, such as VEGF-B, VEGF-C, VEGF-D, and placental growth factor, which, being less potent, have the ability to activate the same receptors. Furthermore, some of these molecules, such as VEGF-C and VEGF-D, not only promote angiogenesis in the normal endothelium, but also doubly affect lymphatic tumor dissemination.6 For these reasons, although this is an attractive and novel new therapeutic target, special attention must be paid to possible adverse effects on patients with hypertension and possible known or unknown tumor pathology. We should have a better knowledge about different molecular mechanisms and pleiotropic effects of VEGF and its possible adverse effects. For these reasons, we believe in the need for future clinical trials of the role of VEGF in the prevention and treatment of hypertension in this population. None." @default.
